Diabetes Center of Excellence

The JCMH Diabetes Center of Excellence provides important education and training.

A Registered Nurse and a Registered Dietitian host diabetes education classes for newly diagnosed diabetic patients and patients who have diabetes and need education and lifestyle information.

The JCMH Diabetes Center of Excellence team works with healthcare professionals on current treatment options, clinical protocols and research data. They teach diabetic patients and family members how to manage the disease in order to live longer, healthier lives.

Individualized education and self-management training is offered in all aspects of diabetes self-care including:

  • Diabetes disease process/self-care

What type of diabetes programs are available?

 Diabetes Self-Management Training

Type 2 Class

This is a group program for those newly-diagnosed with type 2 diabetes or those who have had no training in managing diabetes. This is also a good refresher course for the most current diabetes information. The program includes seven hours of group training with individual follow-up. 

Topics Include:

  • The disease process
  • Self-management and treatment
  • Self blood glucose monitoring
  • Nutrition – eating out and changing recipes
  • Preventing problems
  • Foot care
  • Exercise 

 Gestational Diabetes Program

Women who have been diagnosed with gestational diabetes can take part in an individual session and follow-up with a Registered Dietitian or Registered Nurse.

Topics include:

  • The disease process
  • Goals of control and desired results
  • Self blood glucose monitoring
  • Ketone testing
  • When to alert your doctor
  • Nutrition and exercise

Insulin Pump Program

This program provides training on use of a continuous subcutaneous insulin infusion system (insulin pump). Six to nine hours of training in small group or individual sessions are provided by diabetes specialists who are certified pump trainers. 

Topics include:

  • Pump operation and getting ready for initiation
  • Site care
  • Safety and emergency situations
  • Daily adjustments/instructions
  • Hypoglycemic management
  • Insulin pattern management
  • Carbohydrate counting and meal planning
  • Self blood glucose monitoring

“AIM” for Better Control (Advanced Insulin Management)

“AIM” is a personal program for those ready for advanced training in insulin therapy and who are moving to intensive insulin therapy using basal and bolus insulins with multiple injections.

Topics include:

  • Insulin management
  • Problem solving
  • Carbohydrate counting
  • Nutrition planning
  • Crisis management and preventing problems
  • Self-management

Outpatient Insulin Programs – Diabetes Type 1 or Type 2

These individualized programs are designed for people who have just been diagnosed with type 1 diabetes or for those with type 2 requiring insulin. The basic knowledge and skills needed to begin insulin therapy are covered. 

Topics include:

  • Insulin injection and management
  • Self blood glucose monitoring
  • Ketone testing
  • Nutrition planning
  • Crisis intervention and problems solving
  • Insulin adjustments and supplements
  • Blood glucose pattern control
  • Preventing problems

Diabetes Support Group – No registration Required

All meetings will be held in the JCMH Education Center Conference Room on the 3rd Thursday of each month at 6:30 pm. Call 580-379-5660 for questions about topics.  The Support Group is free and does not require registration. 

How do I get started?

Talk to your doctor about which program is right for you. He or she can then refer you to the JCMH Diabetes Center for education.  The cost of diabetes education is covered by many health insurance companies.
