Confidentiality

JCMH has a strict confidentiality policy regarding patient records, patient rights and protected health information. Ask your nurse or any staff member if you have questions about your personal information.


Patient Satisfaction Survey

After you have received care and have been discharged from a number of services at JCMH, you will receive a patient satisfaction survey in the mail at your home address.

Please fill out the survey and return it in the pre-paid envelope. Include comments about your hospital experience and let us know how we are doing. You are our valued patient and your opinion is very important to us.

One survey is the HCAHPS, Hospital Consumer Assessment of Healthcare Providers and Systems. It is a national, standardized, publicly reported survey of patients’ perspectives regarding their hospital experience. Your participation, by completing the survey and returning it by mail, will help us continue to improve our services.
