JCMH is always seeking to improve our communications and operations in the health care process. As we begin our Patient And Family Advisory Council (PFAC), we want to invite recent patients and/or their family members to apply to join this group.
The PFAC will provide a forum for a small group of patients/family members alongside a small group of employees to share instances in which the hospital could improve communications or processes. Members are expected to participate in monthly meetings.

The mission of the PFAC is to provide a forum for patients and families to participate in decision making, information sharing, and policy and program development; in addition to act as a vital link between JCMH and the community.
WHO CAN JOIN?
Anyone who is committed to pursuing patient safety and quality, and who will be willing to help JCMH continually improve the patient experience is welcome. Applications are available to print below. Call 580-379-5662 to submit application.
MEMBERS WILL BE EXPECTED TO:
• Provide ongoing feedback to address quality, safety and service
• Help JCMH continually improve its services
• Strengthen communications among the whole care team, including the patient and family
• Promote patient and family advocacy and involvement
