How to Nominate an Extraordinary Nurse
DAISY Award honorees personify Jackson County Memorial Hospital’s remarkable patient experience. These nurses consistently demonstrate excellence through their clinical expertise and extraordinary compassionate care, and they are recognized as outstanding role models in our nursing community.
Patients, visitors, and physicians may nominate a deserving nurse by printing and filling out this form and submitting it one of the following ways:
- Place it in one of the many DAISY nomination boxes throughout the facility
- Deliver it directly to one of our Department Leaders
- Mail it to the JCMH Education Department.
Each DAISY Award honoree will be recognized at a public ceremony in her/his unit and will receive a:
- Beautiful Certificate
- DAISY Award Pin
- Hand-Carved Stone Sculpture entitled A Healer’s Touch
Additionally, the honoree will receive Cinnabon® dust and everyone in the unit will celebrate with cinnamon rolls.
Cinnabon® cinnamon rolls were a favorite of Patrick’s during his illness. The Barnes Family asks that whenever and wherever nurses smell that wonderful cinnamon aroma, they stop for a moment and think about how special they are.
