Patient Portal Enrollment

Enroll in the Patient Portal by either completing the online self-enrollment process or by filling out an enrollment form and returning it to the JCMH registration area, the JCMH Health Information Management department or a JCMH Medical Clinic receptionist.

The JCMH Patient Portal gives patients instant access to their health information, including laboratory and radiology results, medications, allergies, demographic information, visit history, discharge/aftercare information, and more. In addition to handling their own care, parents and health care proxies with authorized consent will appreciate the ability to access the records of those individuals for whom they manage care. 

The JCMH Patient Portal allows patients to view the central appointment page which displays their list of scheduled and pending appointments. Here, they can review appointment details.  The patient portal also saves patients time by allowing them to submit updates to their demographic and contact information prior to their visits. The JCMH Patient Portal account is available to all patients age 18 years or older who wish to enroll for access to view their health information.

Each individual requesting access to their information must have a valid, unique email account on file at the hospital or clinic along with a Medical Record number unique to them.  A parent or legal guardian that would like access to their child’s information may do so through proxy access (see Pediatric Proxy Access information).  At the present time we do not offer access to patient information via the portal for children that are ages 13 to 17 years old.

This website is best viewed in Internet Explorer 9 and above, Google Chrome and Firefox. It will not work with older versions. Users assume sole responsibility for configuration, security or content of their particular device. Jackson County Memorial Hospital assumes no liability in this regard.
