MEDICAL RECORDS - Health Information Management

Patients or their personal representative, may inspect or obtain copies of their Jackson County Memorial Hospital medical records or have copies sent to another medical facility. If you would like to obtain a copy of your hospital medical records, print the Authorization for Access by Patient or Disclosure of Protected Health Information Form and have it properly filled out and completed with the appropriate signature. You must also bring a photo I.D.

The Health Information Management department is open Monday through Friday 7:30 am - 5 pm.
