MEDICAL RECORDS - Health Information Management

Patients, or their personal representative, may inspect or obtain copies of their Jackson County Memorial Hospital medical record or have copies sent to another medical facility.

If you would like to obtain a copy of your hospital medical records, print the Authorization for Access by Patient or Disclosure of Protected Health Information Form. Bring the completed, signed form along with a photo I.D.

The Health Information Management department is open Monday through Friday 7:30 am - 5 pm.
